Population Health / Care Management Nurse

The Population Health / Care Management Nurse is responsible for the care of individual patients and populations with chronic disease, primarily through patient education, support and by improving coordination of internal and external communication among provider entities. 
 
Responsibilities include but are not limited to:
  • Provide care management services to patients with chronic disease. Collaborate with the patient, physician, and other care team members in assessing the patient’s progress toward individual healthcare goals, including assisting with removing barriers when patients are not meeting treatment goals, not following treatment care planning, or have not kept important appointments.
  • Apply principles of the Patient-Centered Medical Home to care management role, through use of population-based protocols and evidence-based treatment and management guidelines.
  • Develop a workflow to ensure smooth transition of care for patients treated in another facility (inpatient or emergency department).
  • Collaborate with payer case/care managers for additional services when appropriate.
  • Assure effective group and individual educational classes and support groups are provided for at risk individuals; identify and coordinate with community resources.
  • Submit periodic reports (quantitative and narrative) on measures of activity and outcomes;
  • Collaborate on organizational improvements in clinical process and outcome measures, compliance with standards of care and clinical guidelines.
JOB REQUIREMENTS:
  • LPN or RN (preferred) with active license in State of Ohio and current CPR certification.
  • 3+ years of experience managing patient care processes/programs.  Prefer experience in ambulatory care or clinical health setting with working knowledge of healthcare infrastructure.
  • Proficiency with Microsoft Office programs (especially Excel) required.
  • Experience as a health educator strongly preferred.
  • This position requires work in a clinical office setting.
